Nursing homes are used to providing comfort for their residents. And they respond to the needs of seriously ill people. We want to enhance that quality care as residents approach the end-stages of life.
The Community Hospice wants to extend the services you provide. Once a resident is on our Hospice program, we provide a team to support the patient in cooperation with your staff.
Our board-certified physicians and experienced nurses can help pain and symptom management – enough that many of our patients are actually more functional than when they were undergoing curative treatment.
Our social workers and chaplains can help patients and their families with a variety of services – allowing you to concentrate resources elsewhere. Our volunteers provide companionship, as they sit and chat, or hold a hand, or comfort a patient, allowing your volunteers to complete mission-critical tasks.
We don't replace you. We augment you.
